Learning Disability Support
Tunbridge Wells Primary Care Network is committed to offering reasonable adjustments to all our learning disability patients. The wellbeing and the views of our patients are at the core of our Learning Disability policies and processes, and we are proud to have achieved the highest completion rate of Annual Health Checks in Kent in 2025.
All people with a learning disability from the age of 14 are entitled to an annual health check at their GP Practice. If you have a learning disability and would like a health check, please contact your surgery to book an appointment.
You will be offered a longer appointment with our Learning Disability Nurse who will give you time to say what is important to you about your health. We will work with you to make sure any reasonable adjustments are in place so you can access the health care you need. You will usually have a follow up with GP afterwards.
We will complete a Health Action Plan with you so that you have a record of any actions agreed at the appointment.
We will phone you to remind you of your appointment the day before.
