Learning Disability Week
The theme for 2026 is "Do you see me?" which is all about people with a learning disability being seen, heard and valued.
To mark the occasion, Tunbridge Wells PCN will be sharing a special bulletin with all 8 practices with posters to be displayed on practice boards, showcasing the efforts and dedication of our Learning Disability teams across our practices.
- 100% of all patients (over 14 years of age) with a registered learning disability were offered appointments to complete their Annual Health Checks and Health Action Plans.
- For the second year running, Tunbridge Wells PCN achieved the highest completion rate of Annual Health Checks amongst all 46 PCNs in Kent in 2026.
- In December 2025, all 8 surgeries in our PCN were given the Kent and Medway Learning Disability Annual Health Checks Award, for delivering high-quality Annual Health Checks and Health Action Plans for people with learning disabilities.
- In March we uploaded a dedicated Learning Disability page on our website with useful information to support our patients, their carers and their loved ones.
- We revised the Easy Read pre-questionnaire and invite letter with direct input from our patients feedback.
- We now have the results of the feedback from learning disability patients who attended their annual health checks in 2025-2026, showing a 100% satisfaction rate.
